We have been using the anticoagulation module from Pharmafile since our INR Clinic started in January 2008. We decided to use Pharmafile as our clinical decision support tool mainly because it was a Canadian company
that would give us the most accurate information for our patients. Now, 10 months after starting our clinic, we see approximately 60 patients per day. Although our patients do love the dosing handouts with the Canadian tablet colours, the ease with which we are able to calculate dosage changes is what makes this program indispensable to us.
The list of patients helps us to keep track of who is coming to clinic on a specific day, and which patients are late returning to the Clinic. The clinic workload stats lets us know when we will be at capacity for each day. We have been able to have reports created that are tailored to our needs. And the stats available help us to determine how well we are
doing.
I do not believe that our clinic would run as well as it currently does without this program!

I find Pharma-File is the right tool in the clinical tool box for long term care pharmacy. Warfarin is listed in the State Operations Manual Appendix PP – Guidance to Survey Long Term Care Facilities twenty seven times and INR is listed sixteen times. Clearly since warfarin is the highest mentioned medication in these regulations, it is of great concern to nursing home administrators and Directors of Nursing that the pharmacy can reduce this burden of so much over site to the physicians and their nursing home staff while improving resident care and documentation.
As a long term care pharmacy servicing mostly nursing homes we found our self buried in our own forms. One of our biggest problems was keeping track of who was on service and who was off do to revolving doors between out nursing facilities and hospitals. Pharma-File takes the burden off the pharmacists, physicians and nursing staff by tracking the upcoming lab work due and not losing any of the patients in the shuffle. Their application allows us to make better dosing choices based on long tracking histories of INRs and platelets counts. Pharma-File has helped the nursing staff by being able to receive reports of the upcoming lab work to schedule labs on the correct days and being able to track and report missed labs and resident with compliance issues. Pharma-File is a cost efficient application that quickly pays for it self, by reduced pharmacist time spent daily on dosing and tracking missed received labs. We like the ability to install on every work station and even on our home computers with no extra charges, thus not disturbing the normal work flow of the pharmacy. We like the added feature of being able to fax directly from Pharma-Files fax server saving having to print and feed the fax machine. It tracks all faxes so that there can be no arguments over if received or not. Its ability to save all recommendations to PDF format which truly allows us to work paper free.

I am a cardiologist in a 10-person cardiology group in Los Angeles. We follow approximately 1000 patients in our Coumadin clinic. We have been using using the Pharmafile Coumadin management program for almost 2 years and have been delighted with it. The Pharmafile programmers have been very responsive to our particular needs and suggestions and have been able to tailor the program to fit in with the way we like to run our clinic. Its menu driven commands make it simple to use. The data entry is visually well laid out. The dosing schedules are easy to manipulate and change according to the INR values that are entered. By using built-in algorithms it takes the guesswork out of calculating doses, making it safer for our patients. By automating much of the information it makes us more efficient, and shortens the time we need to care for patients. We particularly like the daily dosage sheets, which patients like a lot as they find them easy to read and follow.

We have been using the Pharmafile warfarin dosing software for several months. It allows us to manage more patients in a more efficient manner. It helped us create a more uniform standard of care among all the pharmacists that are managing warfarin patients.
It is an excellent decision support tool and facilitates communications between our pharmacists, treating physicians, nurses, and of course the patient.
We find particularly useful the automatic daily tablet regiment that we print for patients. It is a clear, concise document which is easy for the patient to understand. We also like the automatic fax server which allows us to automatically fax reports to recipients without having to print them and monitor the fax status.
But the feature which has the most impact on our work flow is the fact that we can access patient files from any computer in the clinic, or outside the clinic. I am the proprietor of three community pharmacies and that specific feature allows me to follow my patients from wherever I am.
I would recommend Pharmafile to anyone serious about Warfarin Management

Our team of community pharmacists uses Pharmafile Warfarin Dosing for our patients and we are delighted with it. It is user friendly, complet and allows us to see all pertinent information at a glance.
As well, the different reports that are available allow us to communicate with all providers involved in our patients' care. The electronic fax transmission is a must. It saves a lot of time, ressources, and is good for the environment!
